Choose the Plan with the benefits You Want


With Assurant Health’s low cost plans, you have options for health coverage — from basic to more enhanced benefits. Plus your employees can have first dollar benefits if you choose the Set Amount benefit plan. Both PPO and traditional plans are available.

You get access to Provider and Hospital networks with discounts of up to 60%. Also, Wellness Services are available, for preventive screening, routine annual physical exam, immunizations, mammograms, and lab/x-ray.


Designed for relatively healthy groups who typically don’t use most of the benefits they pay for, low cost plans encourage your employees and their families to stay healthy by covering routine annual physicals, immunizations, and well-child care benefits.

It also provides you with the flexibility to expand coverage with optional medical benefits such as an outpatient prescription drug copay plan and first-dollar supplemental accident coverage. You can even offer employees a choice of plans through the Employee Choice Program.

Employee Choice is a program which allows you to select up to four plans with variable features to meet different employee needs. This program is well-suited for employers with different levels of employees or those with employees who live outside the area serviced by the primary PPO. The features which you can vary are:
